Position Overview:

Greenlife Healthcare Staffing is seeking a compassionate Licensed Practical Nurse (LPN)  to provide essential nursing care at the Valley View Center for Nursing Care and Rehabilitation  in Goshen, NY. This per-diem role offers flexibility and the opportunity to support patient recovery in a collaborative, mission-driven environment under the supervision of Registered Nurses (RNs).

Qualifications:

  • Education:  Diploma/Certificate from an accredited LPN program.
  • Licensure:  Active New York State LPN License; Basic Life Support (BLS) Certification.
  • Experience:  Prior experience in rehabilitation, long-term care, or similar settings preferred.
  • Technical Skills:  Proficiency in vital signs monitoring, medication administration, and EHR documentation.
  • Soft Skills:  Strong communication, physical stamina, empathy, and attention to detail.

Key Responsibilities:

  • Provide direct patient care, including administering medications and monitoring vital signs.
  • Assist residents with activities of daily living (ADLs): bathing, dressing, feeding, and mobility.
  • Report changes in patient conditions to RNs and physicians promptly.
  • Maintain accurate and timely documentation of care plans and patient progress.
  • Participate in care planning meetings and family conferences.
  • Ensure compliance with facility safety and hygiene protocols.
