Qualifications:
- Education: Diploma/Certificate from an accredited LPN program.
- Licensure: Active New York State LPN License; Basic Life Support (BLS) Certification.
- Experience: Prior experience in rehabilitation, long-term care, or similar settings preferred.
- Technical Skills: Proficiency in vital signs monitoring, medication administration, and EHR documentation.
- Soft Skills: Strong communication, physical stamina, empathy, and attention to detail.
Key Responsibilities:
- Provide direct patient care, including administering medications and monitoring vital signs.
- Assist residents with activities of daily living (ADLs): bathing, dressing, feeding, and mobility.
- Report changes in patient conditions to RNs and physicians promptly.
- Maintain accurate and timely documentation of care plans and patient progress.
- Participate in care planning meetings and family conferences.
- Ensure compliance with facility safety and hygiene protocols.
