Position Overview:

Licensed Practical Nurse (LPN)

to provide essential nursing care at the

Valley View Center for Nursing Care and Rehabilitation

in Goshen, NY. This per-diem role offers flexibility and the opportunity to support patient recovery in a collaborative, mission-driven environment under the supervision of Registered Nurses (RNs).

Education:

Diploma/Certificate from an accredited LPN program. Licensure:

Active New York State LPN License; Basic Life Support (BLS) Certification. Experience:

Prior experience in rehabilitation, long-term care, or similar settings preferred. Technical Skills:

Proficiency in vital signs monitoring, medication administration, and EHR documentation. Soft Skills:

Strong communication, physical stamina, empathy, and attention to detail. Key Responsibilities:

Provide direct patient care, including administering medications and monitoring vital signs. Assist residents with activities of daily living (ADLs): bathing, dressing, feeding, and mobility. Report changes in patient conditions to RNs and physicians promptly. Maintain accurate and timely documentation of care plans and patient progress. Participate in care planning meetings and family conferences. Ensure compliance with facility safety and hygiene protocols.
