Job Description:
The Social Worker is responsible for providing a variety of direct and related services to patients, their families, and the professional staff. Incumbent is guided by a thorough knowledge of the psychosocial evaluation process, social work intervention, rules & regulations of the Dept. of Health, Education, Welfare, and the rules & regulations of the hospital.
Responsibilities: Conducts daily intakes for patients discharged from Psych ED and/or Behavioral Health inpatient units Ensures all patients discharged from Psych ED are connected to care; Works in conjunction with hospital staff to facilitate transitions of care Provides brief follow-up intervention services until connections in the community are made Carries a caseload; Provides individual and group psychotherapy as dictated by the treatment plan Participates in interdisciplinary rounds and teams meetings Participates in hospital wide committees as indicated Participates in quality assurance and performance improvement activities as required Documents service provision as per applicable standards in the clinical EMR Other duties as assigned Requirements:
Master's Degree from an accredited Master of Social Work Program Licensed Master Social Worker (LMSW)/Licensed Clinical Social Worker (LCSW) preferred Experience working with psychiatric population, preferably with experience in psychiatric social work inpatient, psychiatric emergency department, or community crisis and response team Skilled in crisis intervention Experience using evidenced based practices with patients demonstrating suicidal ideation or behaviors Demonstrates a capacity to work collaboratively in a team setting, with an ability to listen to others, participate actively, and take appropriate responsibilities Demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively responds to changing patient needs by making decisions based on ethical principles and adhering to high standards of excellence Bilingual English/Spanish preferred
