Responsibilities:
Educate individuals and communities on various health topics and resources
Conduct outreach activities to promote health programs and services
Assist individuals in navigating the healthcare system and accessing appropriate resources
Provide social support and advocacy for individuals facing health challenges
Collaborate with healthcare professionals to coordinate care for clients
Conduct home visits to assess needs and provide health education
Use motivational interviewing techniques to encourage behavior change
Collect data and maintain accurate records of client interactions
Manage caseload and incoming referrals from health plans
Connect individuals with resources including health, housing and social services - provide case management

Pay: $25 - $30 per hour
Expected hours: Full time M-F 8-3pm (30 Hours per week)
Qualifications: Community Health Worker Certification (Not required but preferred) Bachelor's Degree in Social Work, Psychology, or Sociology and 2 years of experience Community Health Space
Work Location: In person/Remote (Hybrid)
