The requirement is 48 hours of on-call on a weekend and 4 nights during the week - Monday to Friday, 5 p.m. to 8 a.m.
St. Marys Home Care & Hospice in Bucks County, PA, is seeking a Contingent Hospice RN Case Manager for after-hours and weekend coverage as needed.

Position Summary:
The Hospice RN Case Manager provides professional nursing care to hospice clients as prescribed by the physician and in compliance with relevant regulations and policies. Responsibilities include client assessment, care planning, therapeutic interventions, and supervision of care outcomes. The RN collaborates with physicians and the interdisciplinary team to manage patient symptoms, address family grief issues, and implement the hospice care plan. Support is also provided to Hospice Aides and LPNs/LVNs.
The role involves case managing a team of hospice patients, participating in weekly interdisciplinary meetings, and interacting with physicians or hospice medical directors as needed. The position is primarily Monday-Friday, with occasional weekend coverage.
Minimum Qualifications:
- Graduate of an approved nursing program
- RN licensure in Pennsylvania
- At least 1 year of experience as an acute care nurse
- Home care experience preferred
