- Monday, Wednesday, Friday schedule 9am - 4:30pm
- 8 - 13 patients per day
- Patient age range: 5 - 99 years
- Experience with physical/annual wellness assessments
- Focus on chronic conditions management
- Hospital privileges available
- 2 years of NP/PA experience required
- BLS certification required
