5:00 PM Compensation: $81,000
$90,000 annually, based on experience Job Summary: This position involves providing brief, structured mental health therapy (1012 sessions) to adult patients in a primary care environment. As part of a collaborative care team, the clinician addresses mild to moderate mental health conditions by delivering solution-focused, evidence-based support within an integrated health setting. Core Duties: Perform assessments and use targeted treatment methods like behavioral activation and problem-solving therapy. Consult with primary care teams on behavioral health needs and coordinate patient care. Identify mental health and substance use concerns through screening. Facilitate community referrals when additional resources are required. Participate in team meetings and coordinate care across disciplines. Lead groups addressing mental and physical health issues such as diabetes and depression. Maintain accurate documentation using electronic health records. Follow all legal, clinical, and ethical guidelines for service delivery.
