5:00 PM Salary Range: $81,000
$90,000, depending on experience Key Responsibilities: Conduct comprehensive assessments and deliver brief therapy using evidence-based approaches such as behavioral activation and problem-solving therapy. Support primary care providers with mental health consultations and collaborate on patient care plans. Screen adults and older adults for behavioral health needs, including substance use, trauma, and co-occurring disorders. Provide referrals and linkages to appropriate community services when necessary. Participate in interdisciplinary team meetings, case reviews, and care coordination. Facilitate therapeutic or psychoeducational groups targeting mental and chronic health conditions (e.g., depression, diabetes). Ensure timely and accurate documentation in the electronic health record. Maintain compliance with state/federal laws and ethical standards for clinical care.
