The LVN plays a critical role in delivering high-quality care to residents while supporting the interdisciplinary healthcare team. This position is ideal for a nurse who values teamwork, professionalism, and patient-centered care in a long-term and post-acute setting.Key Responsibilities:Provide direct nursing care to residents in accordance with their care plans and physician ordersAdminister medications and treatments accurately and on timeMonitor resident conditions and promptly report changes to the RN or physicianDocument care provided, observations, and medications in compliance with state regulationsAssist with wound care, IV therapy, and infection control measures as appropriateEnsure resident safety, comfort, and dignity in a respectful and supportive environmentCollaborate with CNAs, RNs, physicians, and other care team membersEducate residents and families on care procedures and health maintenanceComply with facility policies, procedures, and state/federal regulatory standardsQualifications:Current California LVN license in good standingCPR/BLS certification requiredPrior experience in a skilled nursing or post-acute care facility preferredStrong communication, critical thinking, and clinical skillsAbility to work effectively in a fast-paced, team-oriented environmentCompassionate and professional bedside mannerWork Environment:Long-term care and rehabilitation settingRotating shifts may include days, evenings, weekends, or holidaysPhysical requirements include standing, walking, lifting, and assisting residents
