Job Summary:

Thousand Oaks Post Acute is seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our skilled nursing team. The LVN plays a critical role in delivering high-quality care to residents while supporting the interdisciplinary healthcare team. This position is ideal for a nurse who values teamwork, professionalism, and patient-centered care in a long-term and post-acute setting.

Key Responsibilities: Provide direct nursing care to residents in accordance with their care plans and physician orders

Administer medications and treatments accurately and on time Monitor resident conditions and promptly report changes to the RN or physician Document care provided, observations, and medications in compliance with state regulations Assist with wound care, IV therapy, and infection control measures as appropriate Ensure resident safety, comfort, and dignity in a respectful and supportive environment Collaborate with CNAs, RNs, physicians, and other care team members Educate residents and families on care procedures and health maintenance Comply with facility policies, procedures, and state/federal regulatory standards

Qualifications: Current California LVN license in good standing CPR/BLS certification required Prior experience in a skilled nursing or post-acute care facility preferred Strong communication, critical thinking, and clinical skills Ability to work effectively in a fast-paced, team-oriented environment Compassionate and professional bedside manner

Work Environment: Long-term care and rehabilitation setting Rotating shifts may include days, evenings, weekends, or holidays Physical requirements include standing, walking, lifting, and assisting residents
