This role encompasses rendering skilled nursing and palliative care services, planning and monitoring in-home services, and assisting in the supervision of home care aides, while also coordinating quality improvement development, support, data collection, and analysis.Essential Job Functions:Provide direct patient care in home and at clinic based upon policy and procedures standards of care and regulatory bodies.Conduct initial comprehensive assessments, unscheduled nursing assessments and participates in IDT.Responsible for the skilled nursing care, palliative care, and home care needs of the group of participants assigned.Facilitates the integration of new participants into the Bluestem PACE health care delivery system including medication, immunizations, and routine monitoring of chronic problems through the health record and nursing care plan development.Provides ongoing nursing assessments on a scheduled basis and submits PACE data requirements for caseload participants.Provides liaison with primary care provider in the event of an episodic illness; assists in coordinating services provided by primary care provider.Assists participants in maintaining optimum health; provides health education and counseling to participants and caregivers facing chronic conditions and end of life issues.Reports changes in the participants' conditions to a physician or responsible party promptly and takes follow-up action as necessary.Prepares and administers medications, enteral nutrition, parenteral nutrition and treatment as ordered.Responsible for the completion of care plans utilizing the nursing process of assessment, establishing goals, planning interventions, and evaluations for the assigned participants.Responsible for a documented encounter at least monthly on assigned participants.Responsible for the completion of the supervisory visits on the participants assigned and notifying Clinical Service Director if care issues arise.Assures nursing services are documented in a timely, legible and accurate manner.Assure that home care charts are up to date.Document care in the participant's medical record consistent with documentation policies and workflows.Follows established standards of nursing practices and implement program policies and procedures.Follows, supervises and evaluates the implementation of the Participant's Bill of Rights.Assumes responsibility for compliance with federal, state and local regulations.Actively participates in Case Management.Provides updates to team regarding participants' change in health or functional status.Participates in monthly quality assurance.Responsible for participant safety, identifying safety hazards and emergency situations and initiates corrective action as able.Assists in assuring personal and staff compliance with infection control, personal protection equipment, blood borne pathogens, hazardous material handling and fire safety.May provide basic first aid to agency employees as required by incident or injury.Attend and participates in staff meetings, in-services, projects and committees as assigned.Follows Bluestem PACE admission, discharge, and transfer procedures and coordinate admissions, discharges and transfers with other departments, physicians, and ancillary service providers. Must be flexible in schedule of hours worked.Provide off-hour on-call assistance as assigned for the completion of skilled nursing and palliative care visits occurring at that time.Requires the use of personal vehicle.Qualifications and Experience:Must be at least eighteen (18) years of ageKansas Nurse degree from an accredited school of nursing or college/universityMust have working knowledge of Microsoft Office SuitePhysical Requirements:Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly.
