Position Overview:
Under the supervision of the Operations Director, oversees the day-to-day operations of the PACE Clinic and Home Care services.
Clinic:Manages the flow of the clinic; leads all staff in the provision of medical and nursing care; maintains the structure and function of nursing case management and caseloads; and ensures high quality standards. Coordinates services with primary care team and acts as primary liaison. Maintains a participant caseload, triages and assesses participants as needed. Acts as a gatekeeper for daily clinic scheduling. Performs participant assessments, care planning, triage and skilled nursing care. Serves to support clinic activities to include implementing orders; care coordination; providing participant, caregiver, family and staff teaching; and communicates participant's condition to caregivers, primary care, and the interdisciplinary team. Has direct oversight of all responsibilities for the RN's, LPN's, CNAs in the clinic.
Home Care:Directs and supervises the Home Care Nurses, CNAs, Non-Skilled/Certified Aides, and Scheduler. When appropriate, performs assessments, clinical intervention and health education for participants/caregivers in their home. Shall act as the liaison between Bluestem PACE and contracted home health agencies.
Essential Job Functions:
Demonstrate effective leadership in managing daily operations of Clinic and Home Care services. Provide direct oversight, clinical education, training/observation, orientation, initial and annual competencies, and evaluation of RNs, LPNs, CNAs and support positions. Ensure that the job responsibilities, authorities, and accountability of direct reports are defined and understood. Ensures that all direct reports provide services based on each participant's plan of care and work within their scope of practice. Communicate effectively with the Operations Director, Medical Director and the Executive Director to keep them abreast of any issues or concerns. Engage in team building, coaching, and mentoring to promote team culture, quality outcomes, autonomy and satisfaction. Evaluate overall effectiveness of the clinic and home care, implementing change and quality improvement as needed. Take lead in effectively communicating with interdisciplinary team members (internally) and contracted providers (externally) at meetings and throughout the course of care delivery. Provide oversight of, and as necessary, conduct in-person comprehensive initial assessments, regularly scheduled reassessments and as needed nursing assessments of PACE enrollees, according to Bluestem PACE protocols. Participates as a member of the interdisciplinary team. Maintains regular attendance at, and participates in, all interdisciplinary team meetings to communicate participant changes, collaborate on care planning decisions and participate in the coordination of 24-hour care delivery. Oversees and ensures signing off orders, ordering medications from pharmacy and disseminating orders and instructions to appropriate personnel. Ensures implementation of physician orders. Assure development and maintenance of nursing standards, policies, and procedures based on established nursing knowledge and research in accordance with all applicable federal, state, local professional and regulatory requirements. Coordinate and participate in the on-call rotation. Manage flow of clinic, oversees appointment bookings, triaging and gate keeping in conjunction with primary care and clinic staff. Ensures timely and appropriate care of participants. Oversee or perform initial assessments of participants in their home environment to determine their individual needs and coordinate plan of care with care-giving resources. Coordinate with the interdisciplinary team to develop care plan for each participant. Assure that home health nursing staff provides education and training to participants and their families on an individual or group basis as needed. Manage and prioritize the utilization of in-home services, in collaboration with the interdisciplinary team, to reflect the assessed needs of all participants. Ensure all contracted home health agency staff complete initial and annual competency testing, and fulfill all CMS and Bluestem PACE training and personnel requirements. Coordinate participant care with outside contracted service providers - hospitals, nursing homes, assisted livings, lab, DME, oxygen, etc. Manage and review participants' records, charts, pertinent information and staff documentation. Document as needed in medical records according to established documentation standards. Maintain fiscal accountability for Clinic and Home Care operations. Participate in and supports Quality Improvement initiatives. Qualifications and Experience:
Must be at least eighteen [18]-years of age Must have Kansas RN license Prefer at least 2 years in a nursing leadership/management position Must have current driver's license and proof of auto insurance
Physical Requirements:
Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly.
