Position Title:
Senior Care Navigator
Work Location:
Remote - Florida (Tampa, Central FL, and Miami)
Assignment Duration:
3 months
Work Schedule:
8am-5pm ET, no OT

Position Summary:
Assesses, plans, implements, and coordinates care management activities based on member needs to provide quality, cost-effective healthcare outcomes. Key Responsibilities: Intake assessments on CMS members (telephonic case management) Set tasking of assigned intake caseload with membership between 150 - 200 members Virtual with a couple of in person meetings per year Predominantly tier 4
Qualification & Experience:
Required: As long as have LPN- no EDU minimum Required: LPN active in Florida Must haves: Pediatrics - Minimum of 2 years Nice to haves: Bilingual preferred/highly encouraged, Experience with electronic medical records, Basic Excel skills Disqualifiers: Non licensure Performance indicators: Caseload compliance Top 3 must-have hard skills: Technology including Microsoft and EMR Problem solving Resource providing
