Responsibilities include: assisting in developing and revising the care plan, providing nursing care per the physicians treatment plan and clients payer authorization, monitoring, recording, and reporting treatment reactions and changes in condition to relevant personnel, providing ongoing instruction to clients, families, and healthcare team members, recording patient condition and care services in electronic charts, and participating in educational programs, quality assessments, and staff meetings. Requirements include: active licensed practical nurse, problem solving skills and ability to multi-task, excellent communication skills, and positive, professional attitude.
