Community Health Policy Manager
Direct community health policy, community-based health initiatives and health programs aimed at promoting health and preventing disease. Implement strategic initiatives to address needs identified in Community Health Needs Assessment. Develop and implement programming to increase access to transitional care and community-based interventions for high-need patients. Develop and implement programming to support integration of behavioral health services into transitional care programs. Identify funding sources, develop and submit proposals to external funders to enhance community-based programs and resources. Education: High School diploma equivalency with 5 years of applicable cumulative job specific experience required, with 2 of those years being in leadership/management OR Associate's degree/Bachelor's degree with 3 years of applicable cumulative job specific experience required, with 2 of those years being in leadership/management. Additional Preferences: BSN or higher degree in health care related field strongly preferred Experience in health care operations Experience in community health, health equity and program development Experience with medical access programs, health equity grants Strong project management skills Ascension St. John has been serving Northeast Oklahoma with compassionate, personalized care since 1926.
