Hours: This position is Casual and will work on Saturdays / Sundays Are you passionate about making a difference in people's lives? We are seeking a dedicated Social Worker to provide comprehensive psychosocial assessments, crisis intervention, and counseling for individuals, families, and groups. Your role will include bereavement and end-of-life support, as well as timely discharge planning to meet anticipated needs. Responsibilities: Provide resource-specific information and facilitate referrals for patients and families. Mentor social work students and serve as a field instructor for MSW students. Utilize therapeutic counseling methods, including crisis intervention and psychosocial support. Educate patients and families on reimbursement systems and advocate for continued care authorizations. Assess and intervene with medically and psychosocially complex patients. Monitor and reassess the plan of care throughout the hospital stay. Collaborate with the healthcare team and community resources to develop individualized care plans. Educate and advocate for patients and families on medical care decisions, discharge planning, and end-of-life care. Complete comprehensive psychosocial assessments considering various factors and support systems. MSW degree from a CSWE accredited School of Social Work required. Licensure, Certifications, and Clearances: Current / active Social Work License required or successful pass of the social work licensing exam within 9 months of employment.
