Certifications: CPR, BLS Must-Have: - Recent specialty experience - Proficiency in HomeCare HomeBase within the last 6 months Description: Provides coordination and direction for home nursing care according to physician's orders. Applies the nursing process competently in accordance with agency policies and procedures. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses. Travel required for home visits and periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Expected to see between 5 to 10 patients per day, equating to approximately 6-8 points. Types of cases include Start of Care, Wound Care, and Follow-up Visits. No guaranteed hours; may need to float to another location if not meeting financial needs due to low patient census. All documentation must be completed within 24 hours of the appointment. Must accept all patients on the schedule regardless of location. Comfortable with travel up to 75 miles from the assigned care center. Local candidates are accepted at the same rate.
