This role requires expertise in home health and proficiency in HCHB/OASIS documentation. **Key Responsibilities:** • Deliver quality physical therapy to home health patients. • Complete same•day documentation using Homecare Homebase (HCHB). • Manage a caseload of 20•25 cases weekly, achieving 5•7 productivity points daily. • Attend weekly in•person office meetings. • Be flexible to float and meet weekend requirements. • Conduct patient visits between 8 am • 5 pm, with flexibility for early or late appointments. **Qualifications:** • Active Physical Therapist License. • Basic Life Support (BLS) certification. • 2+ years of home health experience with HCHB/OASIS proficiency. • Experience with Oasis E documentation post•January 2023. • Efficient caseload management and productivity. **Additional Information:** • 13•week contract, 40•hour workweek. • Mileage reimbursement available. • Pre•approval required for time off.
