The Position: St. Patrick Center, a ministry of Catholic Charities, is hiring two Case Managers to join our Housing, Employment, and Recovery Opportunities (HERO) program. In this rewarding role, you will engage with clients residing in transitional housing, offering guidance and support tailored to their unique needs. You'll work collaboratively with clients to strengthen their connections with family and community resources while actively monitoring their progress on individualized plans. By coordinating personalized services and fostering relationships with service providers both at home and in the community, you will be instrumental in facilitating their journey towards stability and independence.
