The Position: St. Patrick Center, a ministry of Catholic Charities, is hiring two case manager to join our Grant Per Diem (GPD) Program. This role is essential in supporting veterans who are housed in our Permanent Supportive Housing (PSH) program and those participating in the GPD program's retention initiatives. We are looking for a case manager skilled in completing assessments, creating individualized treatment plans, and coordinating housing services.
