Sunbelt Staffing is hiring for a hybrid Nurse Practitioner (NP) in Long Beach, CA , specializing in home health and telehealth care for high-risk, underserved patients. This role blends in-home assessments and telehealth visits to deliver personalized care and chronic condition management in support of value-based care initiatives.

Youll collaborate with a multidisciplinary care team and leverage your autonomy to provide proactive, compassionate carewith no weekends, call, or holiday shifts required (with potential for on?call backup nurse line support in licensed states).

Patient Population & Role Overview

The Advanced Practice Provider will serve patients across the lifespan, from pediatric through geriatric populations. You will conduct comprehensive and follow-up patient assessments via a combination of in-home visits and virtual telehealth appointments. These visits include, but are not limited to :

Annual Wellness Visits (AWV)

Health Risk Assessments (HRA)

All care is delivered per the guidelines set forth by Medicare, Medicaid , and commercial marketplace healthcare plans we serve. The role emphasizes continuity of careidentifying acute and chronic care needs to support care management and active medical intervention.

This position prioritizes value-based care , leveraging home-based delivery to improve outcomes, reduce healthcare costs, address population health gaps, and maximize patient satisfaction.

Key Responsibilities

Conduct comprehensive in-home and virtual patient assessments

Develop and manage individualized treatment and care plans

Support chronic disease management, medication adherence, and health education

Provide post-discharge follow-up and care coordination

Work Hours

MondayFriday, 8 AM 5 PM

No weekends or holidays (with on?call backup nurse line support in licensed states)

Assignment Details

13-week contract to start

4-week extensions

Probable temp-to-perm

Required Qualifications

Minimum 1 year NP clinical experience as an Advanced Practice Provider, preferably in primary care, hospice, urgent care, or outpatient setting

Current, unrestricted California NP license with no disciplinary actions or restrictions

Willingness to obtain additional state licenses for virtual coverage as needed

Ability to treat patients across the lifespan

Graduate of an accredited Nurse Practitioner program

Board Certification from ANCC or AANP

Family Nurse Practitioner (FNP) for family?practice NPs

PHMNP certification if applicable

DEA registration with prescriptive authority in California

Valid National Provider Identifier (NPI)

Reliable transportation with valid drivers license and insurance for home and nursing facility visits

AHA Basic Life Support (BLS) certification

Strong administrative, organizational, and communication skills

Preferred Qualifications

Knowledge of HCC documentation, ICD?10 coding, Health Risk Assessments, HEDIS, and STAR quality measures

Experience caring across lifespan (pediatric through geriatric)

Previous Medent EHR experience preferred

Case management experience a plus

Bilingual proficiency in applicable areas
