Job Summary
Hiring for a Field Nurse Practitioner to deliver screening and preventive primary care services directly to patients in their homes, community settings, nursing facilities, and pop-up clinics. The Field Nurse Practitioner (NP) provides comprehensive assessments and care coordination, ensuring patients receive the right care where they feel most comfortable.
This role is 100% field-based with in-home visits, community engagement, and travel within a 60-mile territory. The NP focuses on initial assessments only no follow-ups or ongoing care and is supported by a 5-week training program.
Key Responsibilities
- Conduct comprehensive medical evaluations, including history, physical exams, and gap-in-care assessments.
- Address both chronic and acute primary care needs, determine medical urgency, and refer to specialists as appropriate.
- Order and interpret diagnostic tests (labs, imaging) as necessary and within resource constraints.
- Create and implement medical plans of care, prescribe medications, and perform procedures within scope.
- Provide care coordination, including post-discharge planning to reduce readmissions and ER visits.
- Participate in pop-up clinics and community health initiatives.
- Document care promptly in Epic EMR while in the field.
- Cover a 60 mile radius; round trip travel can be up to 120 miles daily (mileage reimbursed)
- Collaborate with other NPs to share best practices and attend regional team meetings.
Schedule Options
- Option 1: 4x10 (Mon Fri), 8 AM 7 PM, 1 Saturday/month (8 9 visits/day).
- Option 2: 5x8 (Mon Sat), mix of 8 5 and 10 7 shifts (6 7 visits/day).
- Approximately 45 minutes per visit with 6 9 visits per day.
Required Qualifications
- Licensed Adult or Family Nurse Practitioner (NP).
- Comfortable with 100% fieldwork (home visits and driving).
- Reside within 20 minutes of assigned territory.
- Ability to attend mandatory 5-week onboarding/training.
- Clear background check.
Preferred Qualifications
- Experience in home health, hospice, or with vulnerable/underserved populations.
