Infinity Hospice Care (IHC) is seeking a dedicated and skilled Hospital Coordinator RN (GIP RN) to serve as a key liaison between hospitals, patients, families, and our exceptional hospice and palliative care teams. What You'll Do

As the face of IHC in contracted hospital settings, the GIP RN plays a vital role in helping patients and their families navigate care transitions with confidence and dignity. In this role, you will: Serve as a clinical bridge for patients transitioning from hospital care to IHC home hospice or Nevada End of Life and Palliative Care (NEVPC) Support inpatient palliative care services and hospice admissions directly from the hospital Educate patients and families on hospice and palliative care services, ensuring clear communication of benefits and resources Collaborate closely with case managers, hospital providers, and social workers to identify the most appropriate care setting Promote IHC and NEVPC programs using marketing materials and referral tools Facilitate discharge planning and transitions to SNFs or home care, ensuring seamless coordination and timely documentation Gather and share key clinical information , ensuring all stakeholders are updated on patient outcomes and feedback Support outreach efforts , including presentations to hospital teams and facility partners Ensure excellence in hospital-to-home transitions and overall patient experience Who You Are

We're looking for a compassionate, organized, and collaborative RN who: Has a current RN license in the state of Nevada (Required) Brings experience in hospital or hospice settings , with a strong understanding of hospice admission criteria Communicates clearly and compassionately with patients, families, and clinical teams Thrives in a dynamic hospital environment and can work independently while building strong relationships Has a working knowledge of discharge planning, transitions of care, and interdisciplinary collaboration Is comfortable using electronic systems and sharing patient updates across teams Why Infinity Hospice Care?

Strong hospice knowledge Outgoing personality, team player and maintain confidentiality Ability to prioritize and be flexible Proven success in healthcare marketing preferred Active nursing license from an accredited program and in good standing with the
