Social Services Case Manager
Schedule/Hours: 40 Hours per Week This position is responsible for: Plans, develops, administers, and evaluates the effectiveness and efficiency of the day-to-day operations and assigned staff providing case management services for the social service programs in such a manner as to ensure maximum benefits to clients, the community, and funding agencies. Ensure programing is administered within established policies and procedures including any government and grant guidelines and Salvation Army policies and guidelines. Builds new relationships with community agencies in the assigned service area and maintains effective working relationships with these community agencies for client referrals. Trains and mentors newly assigned Case Management in the appropriate processes and procedures. Supervises and participates in performing day-to-day activities for the smooth operation of a housing facility. Supervises Housing monitors. Accepts and processes residents staying in lodge/shelter overnight; monitors the activity of residents on an assigned shift ensuring safety, security and compliance with house rules; maintains accurate and complete records and logs containing pertinent information; ensures that lodge/shelter is maintained in a neat and proper order. Key Responsibilities: Interviews applicants to determine eligibility for program enrollment based on established facility guidelines and requirements; conduct needs assessment, obtain pertinent information; refers applicants to other agencies if not appropriate for program. Manages a limited caseload of clients providing case management services and other opportunities for development. Supervises Housing Monitors, ensures shifts are covered in event of a Housing Monitor's absence. Responds to emergencies in a calm manner and initiates appropriate corrective actions in accordance with established policies and procedures. Physical Requirements and Working Conditions: Ability to meet attendance requirements. Ability to read, write, and communicate the English language. Ability to read and comprehend legal terminology in order to determine compliance. Ability to perform public speaking. Ability to calculate figures and analyze mathematical computations with accuracy. Ability to lift or leverage innumerable boxes of food and other essential supplies. Ability to drive to assigned areas, as needed. Work requires the ability to lift and carry and/or push/pull moderately heavy (25-50 lbs.) materials, supplies, and equipment. Moderate mobility; duties are usually performed by combinations of sitting, standing, and walking on a frequent change basis. Working Conditions: Work is performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or discomforts with noise, dust, dirt and the like.
