Cape Cod Hospital seeks a compassionate and skilled
Licensed Clinical Social Worker (LCSW)
to provide psychosocial care in the Emergency Department and other clinical areas. This role requires strong clinical assessment abilities, crisis intervention skills, and knowledge of community resources to support patients and families during critical moments. Key Responsibilities Conduct psychosocial assessments addressing home environment, family dynamics, financial needs, substance use, mental health, and cultural factors. Evaluate and support patients with behavioral health, substance use, or psychosocial crises. Develop safe and effective discharge plans in collaboration with multidisciplinary teams. Provide counseling, crisis intervention, and therapeutic support during hospitalization or trauma. Coordinate referrals to community resources, shelters, treatment facilities, and outpatient services. Advocate for patients navigating insurance, financial aid, and healthcare systems. Collaborate with physicians, psychiatric teams, nurses, and external agencies for timely care. Identify and address risk factors for vulnerable populations (e.g., abuse, neglect, domestic violence). Maintain accurate documentation in the Electronic Health Record (EHR) per hospital policy. Adhere to infection control, safety protocols, and ethical standards (NASW Code of Ethics). Participate in departmental meetings, quality improvement, and committee initiatives. Qualifications Master's degree in Social Work (MSW) from an accredited institution. Active Massachusetts LCSW license (or eligibility for temporary/travel licensure). 1-2 years of hospital-based experience, preferably in the Emergency Department. Strong decision-making skills in high-pressure environments. Proficiency in EHR systems and documentation standards. Knowledge of mental health assessments, crisis stabilization, and substance abuse resources. Availability for evening shifts, rotating weekends, and occasional holidays.
