Overview:

We are seeking a Licensed Clinical Social Worker to join the Emergency Department team. This role involves providing crisis intervention, assessment, discharge planning, and coordination of care for patients with acute needs. The position requires flexibility, strong clinical skills, and the ability to work collaboratively with a multidisciplinary team.

Required Qualifications: Licensed Clinical Social Worker (LCSW) in Massachusetts. BLS Certification. Crisis Prevention Intervention (CPI) training. Ability to read, write, and communicate effectively in English. Key Responsibilities:

Conduct initial clinical assessments of individuals with acute care needs in accordance with state requirements. Prepare initial treatment plans and communicate findings to the care team. Lead treatment and discharge planning in collaboration with psychiatrists and other providers. Facilitate group therapy, individual counseling, and family counseling sessions. Coordinate patient care planning to ensure continuity and optimal recovery. Assess ongoing medical and psychosocial needs for home care planning and arrange appropriate community services. Link patients to available programs and services for ongoing support post-discharge. Assist with procurement of essential home care equipment. Support patient/family decision-making regarding facility placement when needed. Connect patients and families with community-based resources and serve as a liaison with external agencies. Evaluate and coordinate appropriate community services to meet patient needs. Provide crisis prevention and intervention services as needed, including support to newly discharged patients and caregivers. Provide coverage for Inpatient and Partial Hospital Program Behavioral Health Clinicians when necessary.
