Medical Social Worker
Deaconess Home Health, Jasper, Indiana The posted compensation range of $20.45 - $28.12/hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law. This is a Per Diem position for the Home Health and Hospice department. The shift is Day, and it is PRN with no remote option. Travel is required for 75% of the time. Responsibilities include establishing a care plan in accordance with the physician's plan of treatment, providing crisis intervention as required, assisting the physician and other caregivers in understanding the significant social, emotional, and economic factors influencing the patient's ability to improve medically, providing patient care education to patients and caregivers, assisting in making appropriate community referrals to gain access to additional resources as needed, working with the patient's caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver, and completing and submitting all required documentation in an accurate and timely manner. Qualifications include a Master's degree from an accredited school approved by the Council on Social Work Education, certification under IC25-23.6-4-3 in Indiana, currently licensed to practice in the state serviced, one year social work experience in a health care setting, and current CPR/BLS certification through the American Heart Association or American Red Cross and compliance with state requirements.
