Medical Social Worker
CHI Franciscan Health at Home, University Place, Washington The posted compensation range of $31.56 - $45.77 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law. Responsibilities Establishing a care plan in accordance with the physician's plan of treatment. Providing crisis intervention as required. Assisting the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient's ability to improve medically. Providing patient care education to patients and caregivers. Assisting in making appropriate community referrals to gain access to additional resources as needed (i.e. applications for public assistance, housing problems, additional health care resources, transportation) Working with the patient's caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.
