Position Summary
The Clinical Director / DON is responsible for overseeing all aspects of clinical care delivery within our organization. This role ensures compliance with federal, state, and local regulations, supervises the interdisciplinary group (IDG), and fosters a culture of clinical excellence, compassion, and teamwork.
Key Responsibilities Lead, mentor, and support the clinical team, including RNs, LPNs, aides, social workers, chaplains, and therapists. Direct and coordinate patient care services to ensure quality, safety, and compliance with Medicare Conditions of Participation (CoPs) and state regulations. Oversee the admission, care planning, and discharge processes. Facilitate and participate in IDG meetings to ensure holistic, individualized patient care. Monitor clinical outcomes, patient satisfaction, and quality improvement initiatives. Ensure proper documentation and timely, accurate charting in compliance with hospice and home health regulations. Collaborate with the Agency Administrators to develop and implement policies, procedures, and best practices. Manage clinical staff scheduling to ensure adequate coverage for all patients. Participate in community outreach and represent the organization with professionalism. Qualifications Current RN license in TN (required). Bachelor's degree in Nursing (BSN) required Minimum 3 years of clinical leadership experience in hospice, palliative care, or home health. Strong working knowledge of hospice and/or home health regulations and standards. Exceptional leadership, organizational, and interpersonal skills. Compassionate, patient-focused approach with strong decision-making abilities.
