Job Description:
We are seeking a dedicated Home Health Physical Therapist (PT) to join our dynamic team in Sonoma, CA, and nearby areas. This is an excellent opportunity for a skilled professional to provide one-on-one patient care in a home setting, supporting individuals in achieving their rehabilitation goals.
Responsibilities: - Provide skilled physical therapy services to patients in their homes, ensuring high standards of care and client satisfaction. - Conduct thorough assessments and develop individualized treatment plans based on patient needs and clinical best practices. - Manage a case load of up to 6 patients daily, balancing productivity with quality of care. - Collaborate with other healthcare professionals to ensure comprehensive patient care and effective rehabilitation outcomes. - Maintain accurate and up-to-date patient records and documentation using EPIC/Oasis charting systems. - Educate patients and their families on home exercise programs, injury prevention strategies, and health maintenance.
Qualifications: - Must possess a valid state license as a Physical Therapist. - Minimum of 1 year of skilled Physical Therapy experience is required. - BCLS certification is mandatory. - Experience with Oasis charting is preferred. - Strong interpersonal communication and patient education skills.
Work Environment: - This position is primarily home health care and requires reliable transportation for patient visits. - Free parking available at facilities. - Mileage reimbursement provided at IRS standard rates.
Additional Details: - No on-call requirements for this position. - Traveling professionals should be open to executing assignments in varying locations as needed. - Guaranteed hours may include some flexibility, with the possibility of being called off for one shift every pay period.
