Job Description Job Title: Physical Therapist (Home Health) Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 year of Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience
Description: This position involves providing therapy services in a home health setting. You will conduct home visits for patients and participate in periodic office meetings. Must possess reliable transportation and a current driver's license. Agency-required liability insurance is necessary. The average number of patients per day is 5-6. You will be assigned between 5-10 patients per day, equating to roughly 6-8 points. You are expected to achieve 30 productivity points per week. The types of cases include Start of Care and Follow Ups. Weekend requirements vary by location and profession. You should be willing to travel up to 75 miles from your home base for patient visits. Mileage will be reimbursed at the IRS standard rate. All documentation must be completed within 24 hours of each appointment. There are no guaranteed hours with this position. All clinicians must accept every patient on their schedule, regardless of location. Delayed or incomplete documentation may result in cancellations. Effective communication and time management skills are essential. Ability to work independently and manage a flexible schedule is important. Familiarity with electronic medical records systems is preferred.
