Job Description
Administer skilled nursing care for clients of all ages in their place of residence. Coordinate care with the interdisciplinary team, patient/family, and referring agency. Assess home health and/or hospice patients in stable and emergent situations. Implement, develop, document, and evaluate the Plan of Care (POC). Obtain data on factors influencing patient/family health status. Maintain up-to-date patient records for quality care and documentation. Utilize infection control measures to protect staff and patients. Educate patients and/or families regarding care, diet, medications, and home safety. Supervise and provide clinical direction to HHA/STNA/LVN. Assess and manage pain; educate patients and families on pain management. Arrange referrals to appropriate agencies when necessary. Monitor assigned cases for compliance with third-party payer requirements. Perform on-call responsibilities for patients/families as assigned. Provide effective communication as a liaison to patients, families, and healthcare professionals. Attend interdisciplinary team meetings as necessary. Perform other related duties as assigned or requested.
Critical Information
Knowledge of medications and their correct administration. Current state license as a Registered Nurse is required. 1-2 years experience as a Registered Nurse in a clinical care setting is required. 2 years of Home health experience is required.
Education/Licenses Needed
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program is required. Bachelors of Science in Nursing is preferred. Current CPR certification and driver's license are required.
