Job Description
Administer skilled nursing care for clients of all ages in their place of residence. Coordinate care with the interdisciplinary team, patient/family, and referring agency. Assess home health and/or hospice patients in stable and emergent situations. Implement, develop, document, and evaluate the Plan of Care (POC). Obtain data on factors influencing patient/family health status. Maintain up-to-date patient records. Utilize infection control measures. Educate patients and/or families regarding care, diet, medications, home safety, etc. Supervise and provide clinical directions to the HHA/STNA/LVN. Assess and manage pain. Arrange referrals to appropriate agencies. Monitor assigned cases for compliance with third-party payer requirements. Perform on-call responsibilities as assigned. Provide effective communication and serve as a liaison to patients/family and healthcare professionals. Attend interdisciplinary team meetings when appropriate. Perform other related duties as assigned or requested.
Critical Information
Must have reliable transportation and insurance. 1-2 years experience as a Registered Nurse in a clinical care setting is required. 2 years home health experience is required.
Education/Licenses Needed
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program is required. Bachelors of Science in Nursing is preferred. Current state license as a Registered Nurse is required. Proof of current CPR certification is required. CHPN certification is preferred.
