POSITION SUMMARY
- RN Case Manager
POSITION DUTIES
- Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans in an Inpatient Rehab (with possible float to LTACH)
MINIMUM REQUIRED QUALIFICATIONS
- 1+ year recent acute hospital case management experience
LENGTH OF ASSIGNMENT
- 13 weeks
SHIFT / HOURS PER WEEK
- M-F & at least 2 weekend days per month (Saturday or Sunday)
SYSTEMS
- Linq, Prodoc & Protouch
START DATE
- ASAP
