Position Summary:

Key Responsibilities:
• Provide occupational therapy services in a skilled nursing facility setting • Work Sunday through Thursday schedule from 8:00 AM to 4:30 PM • Assess patients' functional abilities and develop appropriate treatment plans • Implement therapeutic interventions to improve patients' daily living skills • Document patient progress and maintain accurate medical records • Collaborate with interdisciplinary healthcare team members • Educate patients and families on therapeutic techniques and safety measures • Ensure compliance with state regulations and facility policies • Maintain current occupational therapy licensure and certifications • Participate in care plan meetings and discharge planning • Provide skilled rehabilitation services to help patients achieve maximum independence
Job Requirements:
Active, unencumbered license in CA
Minimum of 1-7 years of relevant experience, depending on specialty
Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
Current Resume listing detailed work history (MM/YYYY format)
Two current clinical references
State-required professional license
Physicial exam (completed within the past 12 months)
TB skin test or Quantiferon Gold (within the past 12 months)
Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
Current Tdap and Flu vaccinations
Must pass background check and drug screening
