Job Description Salary: $45,000 starting level POSITION: Redeploy/Second Chance Case Manager

QUALIFICATIONS: Bachelor Degree in behavioral or social sciences. Demonstrated interest in, and ability to, understand the general problems and needs of at-risk youth and their families. Skills and willingness to facilitate group therapy. Sensitivity and responsiveness to the cultural differences of individuals and families in groups outside ones own culture.

CLASSIFICATION: Exempt Full-Time

REPORTS TO: Director of Juvenile Justice Services

Summary: Carries a caseload of clients and provides direct services.

Duties for Direct Service to Youth and Families Accepts case assignment from the Director of Juvenile Justice Services and begins services within the prescribed guidelines. Obtains proper releases and consents, and safeguards the confidentiality of all records. Insures all necessary documentation is properly signed and in the client file. Works with the Redeploy/Second Chance therapist, youth and family to develop a Service Plan outlining service needs. Maintains regular contact with the youth and families to provide case management services including transportation, coordination and referral to other community resources and specialized services as appropriate. Provides counseling to youth and their families. Co-leads group as prescribed. Participates in 24-hour emergency on-call rotation. Leads youth portion of group as prescribed. Participates in all inter-agency staffing's of clients. Attends court dates for assigned clients. Completes assessments with youth.

Support Services Completes case notes, reports, documentation and paperwork as necessary and required, within prescribed timelines. Maintains an on-going case file for each youth which is up to date and contains all required documentation. Participates in all required YSBIV staffing's and team meetings. Develops a working knowledge of, and establishes and cultivates cooperative professional relationships with, community resources, allied agencies, and referral sources.

III. Other Duties Reports itinerary on a daily basis. Completes time sheets on a bi-weekly basis. Attends all staff meetings and training sessions as required. Participates in employee annual evaluation. Represents YSBIV at local community events as directed. Actively participates in YSBIV's Continuous Quality Improvement program.

Communication and Collaboration: The ability to establish rapport with families, engage them in the case planning, and facilitate open and honest communication to address their needs and concerns. Case Coordination and Advocacy: The ability to assess clients' needs, develop individualized service plans, coordinate access to necessary resources (such as counseling, education, vocational training, or housing), and advocate for clients' rights and needs within the juvenile justice system and broader community.
