Functions as a problem solver and facilitator with the primary objective of providing continuity of care to meet the needs of patients and families. The primary role is to serve on the Admission team as a Rapid Responder, obtaining signed consents, processing pending referrals and pending list phone calls, completing non-nursing documentation in any healthcare setting, assisting with the admissions process from Intake to the Field, and responding to patient needs by representing the Trustbridge values. Assists with coordinating the plan of care and treatment (in person or telephonically) for hospice-appropriate patients and their families. Able to articulate all components of the organization's continuum of care, guiding patients and families to necessary services. Qualifications Education/Regulatory Requirements: Bachelor's Degree preferred (in health-related or human services field). Skills: 3 to 5 years of experience in sales/referral representation in healthcare and/or community settings. Strong understanding of medical terminology. Hospice experience highly desirable. Must possess the ability to communicate physical and psychosocial needs of patients and families. Ability to present the hospice program and continuum of care in a positive manner and facilitate a timely admission. Organized, flexible, self-motivated, with the ability to communicate effectively with all levels of a professional medical team. Able to respond to several cases simultaneously, prioritizing continually, yet following up in a timely manner on urgent needs. Able to work independently with minimal supervision. Works effectively under stress and deadlines. Professional Requirements:

Excellent communication skills. Language Skills: Bilingual (English/Spanish) preferred.
