The registered respiratory therapist-ECMO specialist (RRT-ES) provides respiratory care services in accordance with department policies. Administers, monitors, controls, and evaluates complex life support systems that ventilate acute or chronically ill patients. An RRT-ES is a technical specialist trained to manage the ECMO system, including blood pump, tubing, artificial oxygenator, and related equipment. Under qualified medical direction and supervision, the ECMO Specialist is responsible for the clinical needs of the patient on ECMO, which includes:
Maintaining normal acid-base balance, oxygenation, and ventilation
Administering blood and blood by-products
Delivering medications
Maintaining appropriate anticoagulation
The role involves participation in hospital initiatives and projects.
Essential Responsibilities / Duties - Respiratory Therapy:
Conduct respiratory care services according to department policies to ensure quality patient care. Review patient charts and interpret physician’s orders accurately. Set up appropriate equipment and medications for tests and therapies. Administer tests, treatments, and therapies as ordered, documenting all interventions. Operate mechanical ventilators and monitor patients receiving ventilation, including performing suctioning as needed. Perform patient rounds, observe condition, check equipment, and make necessary adjustments. Remove and disassemble equipment following department policies. Recognize adverse reactions, discontinue therapy if needed, and notify physicians immediately. Apply emergency protocols and assist physicians during cardiac arrests. Provide care appropriate for all age groups served, including neonatal, pediatric, adolescent, adult, and geriatric patients. Notify Lead RRT when patients outside regular age groups are present and seek assistance as needed. Participate in developing protocols, policies, and procedures, and provide in-service education. Assist in quality improvement activities, data collection, and analysis. Perform advanced clinical assignments, including pulmonary function testing, tracheotomy changes, blood gas analysis, and intubation following department policies. Maintain clinical competency and current Massachusetts license and NBRC registration. Attend mandatory education and participate in hospital committees and meetings. Manage work efficiently, keep work area organized, and operate equipment safely. Maintain confidentiality and adhere to hospital standards and safety procedures. Responsibilities - ECMO Specialist:
Support and train staff on ECMO procedures, ensure equipment maintenance, and support patient care with ECMO devices. Responsibilities include: Coordinate clinical resources for ECMO use, emphasizing safety. Operate ECMO devices, participate in device support from implantation to removal, and perform routine maintenance. Serve as a consultant for device operation, troubleshooting, and evaluation. Train staff on ECMO device operation and care. Assist with patient transport and bedside care, including crisis interventions. Record data and prepare documentation for regulatory and registry purposes. Maintain knowledge of regulations and technological updates. Provide 24/7 support for ECMO patients. Follow safety protocols and report incidents promptly. Perform ACT analysis and interpretation, requiring an A.S. Degree in Respiratory Therapy and successful completion of an ELSO-approved ECMO course. Education/License/Certification Requirements:
Complete an ELSO-approved or BMC-approved ECMO Specialist course within 3-6 months of start date. Current certification as an ECMO Specialist preferred; attendance at next BMC ECMO course required. Licensed Respiratory Therapist in Massachusetts, registered with NBRC, and active member of the American Association of Respiratory Care preferred. Current BLS certification; ACLS preferred. Experience and Skills:
Minimum of two years as a Respiratory Therapist. Knowledge of medical terminology, anatomy, physiology, microbiology, pulmonary physics, pathophysiology, and pharmacology.
