Boston Medical Center (BMC) . The registered respiratory therapist-ECMO specialist (RRT-ES) provides respiratory care services in accordance with department policies. The ECMO Specialist, under qualified medical direction and supervision, is educated to manage the ECMO system including blood pump, tubing, artificial oxygenator, and related equipment, and is responsible for the clinical needs of the patient on ECMO. This includes maintenance of acid-base balance, oxygenation, ventilation, administration of blood and blood by-products, medication delivery, and maintenance of anticoagulation. Participates in hospital initiatives and projects. Essential responsibilities / Duties – Respiratory Therapy

Conducts Respiratory Care services according to department policy and procedure, ensuring quality of care. Reviews patient charts and interprets physician orders accurately. Set up equipment and medications for tests and therapies; perform therapies as ordered (e.g., oxygen, humidity therapy, airway management, resuscitation, mechanical ventilation). Document every patient treatment and interaction; monitor response to therapy. Operate mechanical ventilators and monitor patients on ventilation; perform endotracheal suctioning as needed. Make patient rounds as scheduled; observe condition, equipment operation, and adjust as necessary; discontinue therapy as ordered. Ensure proper equipment handling, maintenance, and safe removal from patient rooms. Recognize adverse reactions; discontinue treatment when indicated and notify the physician. Apply emergency protocols; assist with airway management in cardiac arrest. Provide care appropriate to ages served (neonatal to geriatric); notify Lead RRT when specialized age groups are present. Participate in development of protocols, clinical pathways, policies, and procedures; provide in-service education as needed. Assist in quality improvement efforts, data collection, analysis, and corrective actions. Attend Advanced Cardiac Life Support as required; assist with bronchoscopy per department policy. Perform pulmonary function testing as directed and arterial blood gas procedures as directed; maintain confidentiality. Maintain current Massachusetts Respiratory Therapist license and NBRC registration; attend mandatory continuing education; assume Lead RRT responsibilities when directed; participate in department/hospital committees. Demonstrate knowledge of patient needs and communicate effectively with the care team; maintain clean and organized work areas; follow infection control and safety procedures. ECMO Specialist Responsibilities

Coordinate clinical resource needs for ECMO use with emphasis on safety. Operate ECMO devices in the clinical environment; participate in device support from implantation through removal; ensure equipment readiness and routine maintenance/calibrations. Act as a consultant for ECMO devices; collaborate with physicians, nurses, and respiratory therapists on patient care. Train clinical staff in ECMO device operation and care; assist with ECMO patient transport and bedside nursing support. Record and prepare documentation required by FDA, device manufacturers, and ELSO registry; communicate equipment issues with vendors. Maintain current knowledge of regulations and standards; provide 24/7 ECMO coverage as needed. Meet minimum qualifications: AS degree in Respiratory Therapy and completion of an ELSO-approved ECMO specialist course. Education / Licensure / Certification

Complete an AELS0-approved or BMC-approved ECMO Specialist course within 3–6 months of start date. Current ECMO Specialist certification preferred; required to attend next available BMC ECMO course. Licensed Respiratory Therapist in Massachusetts; Registered Respiratory Therapist with NBRC; BLS required; ACLS preferred (not required). Experience

Requires a minimum of two years as a Respiratory Therapist. Knowledge and Skills

Basic medical terminology; knowledge of anatomy, physiology, microbiology, pulmonary physics/pathophysiology, respiratory care, pharmacology, and related subjects. Ability to read and communicate in English; strong prioritization and teamwork; excellent interpersonal and communication skills. Caring, empathetic approach; ability to explain therapies to patients and families; solid confidentiality practices.
