Description Conduct evaluations of patients in both inpatient and outpatient environments, crafting personalized treatment plans tailored to individual needs Implement physical therapy techniques aimed at enhancing mobility, strength, balance, and overall function Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to achieve outstanding outcomes Foster education for patients and their families on therapeutic objectives, discharge strategies, and home exercise regimens Maintain accurate documentation of evaluations, treatments, and patient progress in accordance with facility policies and standards Participate actively in interdisciplinary case rounds and conferences to ensure cohesive and coordinated care Education

Doctorate in Physical Therapy (DPT) or a relevant degree from a recognized program Certifications

Valid state licensure as a

Physical Therapist CPR certification preferred Specialty certifications (e.g., neurological, geriatric) are advantageous but not essential Skills

Preferred for candidates to have prior experience Competence in treating patients with complex medical conditions and mobility challenges Excellent communication and teamwork abilities Familiarity with digital charting and clinical documentation systems Benefits
