Description Assess patients in either inpatient or outpatient environments and craft personalized treatment plans Implement physical therapy interventions concentrating on mobility, strength, balance, and functional capabilities Collaborate with physicians, nurses, case managers, and other rehabilitation professionals to achieve optimal outcomes Guide patients and their families on therapeutic objectives, discharge preparations, and home exercise routines Maintain accurate documentation of evaluations, treatments, and patient progress per regulatory and facility standards Engage in care team discussions and interdisciplinary meetings to guarantee synchronized care Education

Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution Certifications

State licensure as a

Physical Therapist CPR certification is preferred Specialty certifications (e.g., neurological, geriatric) are advantageous, though not required Skills

Prior experience is strongly preferred Ability to manage patients with intricate medical and mobility challenges Exceptional communication and teamwork abilities Comfortable utilizing digital charting and clinical documentation systems Benefits
