Salary and Benefits Base salary of $130,000 10% annual performance bonus (based on base salary) Total earning potential of $143,000+ Comprehensive health, dental, and vision insurance Paid time off and holidays Employer-funded retirement plan Flexible field-based schedule Mileage reimbursement Medical equipment provided Responsibilities Deliver high-quality primary care directly in patients’ homes Provide ongoing care for geriatric patients with chronic and complex conditions Coordinate closely with Medical Assistants and interdisciplinary care teams Maintain accurate and timely documentation of visits and care plans Assist in scheduling and identifying patients for home-based visits Participate in quality improvement activities and patient care planning Travel within a defined geographic region using personal transportation Requirements Nurse Practitioner degree from an accredited program Active, unrestricted Nurse Practitioner license in Missouri Minimum 1 year of experience in geriatrics, internal medicine, or family medicine BLS/ACLS certification Proficient in managing chronic conditions in a home-based setting Able to lift and transport up to 20 lbs. of equipment Excellent clinical decision-making and communication skills Reliable vehicle and valid driver’s license About Us Titan Placement Group is a permanent placement healthcare recruiting firm that bridges the gap between mission-driven healthcare organizations and high-quality candidates.
