Part-Time Palliative Care Nurse Practitioner (NP)
Titan Placement Group invites you to explore an opportunity with one of the nation's fastest-growing hospice and palliative care providers. Salary and Benefits
Hourly administrative compensation: $50/hour Per visit rates:
Start of Care Visit: $180 per visit Routine Visit: $90 per visit Face-to-Face Visit: $100 per visit Tele-Med Face-to-Face: $100 per visit Palliative Care to Hospice Transition: $100 per visit
Company gas card or mileage reimbursement provided (employee choice) Potential transition to full-time status after 90 days for high-performing candidates Responsibilities
Provide in-person and virtual palliative care consults across various settings Initiate treatment plans based on comprehensive patient evaluations Prescribe medications and order diagnostics per state laws and organizational protocols Document timely and accurately in the Palliative Care EMR system Conduct face-to-face hospice eligibility assessments for recertification Participate in interdisciplinary team meetings and case reviews as needed Educate patients and families on palliative care goals and care planning Offer guidance to healthcare professionals and staff through education and mentorship Maintain productivity standards and comply with clinical documentation guidelines Self-schedule patient visits to ensure minimum compliance of one visit every 30 days per patient Average 4-6 patients seen per day Complete an average of 10 hours of admin work per week, with 15 hours once per month for IDG meeting preparation Perform other duties as assigned Requirements
Active Nurse Practitioner license in the state of GA Minimum of one year of experience in hospice or home health Experience in establishing and managing a plan of care in the patient's home setting Valid driver's license and reliable, insured transportation Ability to lift up to 25 pounds and perform standard patient care movements Strong communication, critical thinking, and emotional resilience RNs do not report directly to the Nurse Practitioner
