Description:

The Clinical Pharmacist-Antimicrobial Stewardship works with Physician champion to ensure appropriate antimicrobial utilization within the JPS Health Network. Typical Duties: Performs drug utilization reviews, assists in maintaining integrity of formulary and provides review of the antimicrobial pharmacokinetic dosing and monitoring services offered. Monitors antimicrobial resistance patterns among pathogens isolated from patients within the JPS Health Network. Rounds with Infectious Diseases Physicians to optimize therapy and ensure safe and effective medication regimens for patients. Provides education for medical, nursing, pharmacy and other clinical staff as well as patients of JPS Health Network. Serves as a clinical preceptor for pharmacy students and residents. Functions as an Infectious Diseases pharmacotherapy resource for the District staff. Represents the department in interdepartmental/district meetings pertaining to Infectious Diseases and Antimicrobial Stewardship, as required. Participates in the development of medication use management programs within the JPS Health Network including clinical guidelines, disease management and drug use programs to optimize the utilization of systemic antimicrobial agents and provide evidence-based recommendations regarding the appropriateness of antimicrobial agents. Ensures that institutional policies regarding antimicrobial agent use comply with regulatory agency standards. Ensures completion and accuracy of required documentation and record keeping. Performs all other related job duties as assigned.

Qualifications:

Required Education and Experience:

Graduate of accredited college of pharmacy. 1 year American Society of Health-System Pharmacist (ASHP) accredited residency and 4 plus years of acute hospital experience working in infectious diseases patient care area demonstrating direct patient care in medication management and participation in patient rounds with medical and hospital staff.

Preferred Education and Experience:

2 year American Society of Health-System Pharmacist (ASHP) accredited residency in Infectious Diseases.

Required Licensure/Certification/Specialized Training:

Pharmacy license from Texas State Board of Pharmacy or eligible for same. Specialty Board Certification or complete within 24 months of hire. Current JPS recognized Basic Life Support (BLS) certification and must be maintained throughout employment. May be obtained within 3 months of hire. Preceptor with Texas State Board of Pharmacy or complete within 6 months of hire.
