Description Conduct evaluations of patients in both inpatient and outpatient settings while crafting personalized treatment plans Implement targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality Work collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to ensure optimal patient outcomes Inform and educate patients and their families about therapeutic objectives, discharge planning, and exercises to continue at home Maintain thorough documentation of evaluations, treatments, and patient progress in alignment with facility and regulatory standards Participate in interdisciplinary team rounds and conferences to promote coordinated and holistic care Education
Doctorate in Physical Therapy (DPT) or an equivalent advanced degree from an accredited program Certifications
State licensure as a
Physical Therapist CPR certification is preferred Any specialized certifications (such as neurological or geriatric) are advantageous but not mandatory Skills
Prior experience in the field is strongly preferred Capability to care for patients with complex medical histories and mobility challenges Excellent communication and teamwork abilities Familiarity with digital charting and clinical documentation solutions Benefits
