Description Assess patients in both inpatient and outpatient environments and tailor treatment plans to their unique needs Implement physical therapy interventions that enhance mobility, strength, balance, and overall functionality Work collaboratively with a team of nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes Guide patients and their families through therapeutic goals, discharge planning, and home exercise regimens Maintain accurate documentation of evaluations, treatments, and progress in line with facility policies and regulations Engage fully in care team meetings and interdisciplinary discussions to ensure holistic care delivery Education

Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution Certifications

Valid state licensure as a

Physical Therapist CPR certification is preferred Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory Skills

Preference for prior experience in similar roles Proficiency in addressing complex medical and mobility challenges Exceptional communication and teamwork abilities Familiarity with digital charting and clinical documentation systems Benefits
