Description Conduct thorough evaluations of patients in an inpatient or outpatient clinic and formulate tailored treatment plans Administer targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function Collaboratively work with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes Educate patients and their families on therapeutic objectives, discharge procedures, and home exercise regimens Ensure accurate documentation of evaluations, treatments, and progress in line with facility protocols and regulatory standards Engage in care team discussions and interdisciplinary meetings to guarantee a holistic approach to care Education

Doctorate in Physical Therapy (DPT) or comparable degree from an accredited institution Certifications

Current state license as a

Physical Therapist CPR certification is preferred Additional specialized certifications in areas like neurology or geriatrics are advantageous but not required Skills

Prior experience is highly desirable Ability to manage patients with complex medical conditions and mobility challenges Strong collaboration and communication skills Proficient with digital charting and clinical documentation systems Benefits
