Description Evaluate patients in either an inpatient or outpatient setting and create tailored treatment plans Implement physical therapy interventions focused on improving mobility, strength, balance, and daily function Work collaboratively with nurses, physicians, case managers, and rehabilitation experts to optimize patient outcomes Educate patients and their families about therapeutic goals, discharge processes, and recommended home exercises Document evaluations, treatments, and patient progress in line with facility policies and regulatory standards Participate in interdisciplinary team rounds and care conferences to ensure a cohesive approach to patient care Education
Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution Certifications
State licensure as a
Physical Therapist CPR certification is preferred Specialized certifications (such as neurological or geriatric) are beneficial, but not mandatory Skills
Prior experience is highly valued Ability to care for patients with complex medical and mobility needs Exceptional communication and teamwork capabilities Proficient in digital charting and clinical documentation processes Benefits
