Description Conduct comprehensive patient evaluations in both inpatient and outpatient environments while crafting tailored treatment plans Deliver targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality Work in tandem with nurses, physicians, case managers, and a variety of rehabilitation specialists to maximize patient outcomes Educate patients and their families on therapeutic objectives, discharge planning, and at-home exercise regimens Meticulously document assessments, treatments, and patient progress in line with established facility and regulatory standards Engage actively in care team meetings and interdisciplinary conferences to maintain coordinated patient care Education
Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution Certifications
Valid state licensure as a
Physical Therapist Current CPR certification preferred Specialty certifications (for example, neurological or geriatric) are desirable, but not mandatory Skills
Prior experience in a clinical setting highly preferred Proficiency in managing patients with intricate medical and mobility challenges Exceptional communication and team collaboration skills Familiarity with digital charting and clinical documentation systems required Benefits
