Description Conduct assessments of patients in both inpatient and outpatient settings and devise customized treatment plans Implement therapeutic interventions aimed at enhancing mobility, strength, balance, and overall function Engage in collaboration with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimum patient outcomes Guide patients and their families on therapeutic objectives, discharge planning, and home exercise regimens Document evaluations, treatment sessions, and progress according to facility-specific and regulatory standards Take part in care team discussions and interdisciplinary meetings to promote integrated care Education
Doctorate in Physical Therapy (DPT) or equivalent credential from a recognized program Certifications
State licensure as a
Physical Therapist CPR certification is preferred Specialty certifications (such as in neurological or geriatric therapy) are advantageous but not mandatory Skills
Prior experience is strongly preferred Proficiency in treating patients with complex medical and mobility challenges Strong communication and collaborative abilities Comfortable with electronic health records and clinical documentation systems Benefits
